What is Paediatric Epilepsy?

Paediatric epilepsy refers to a medical condition characterised by recurrent seizures in children. Epileptic seizures occur due to abnormal electrical activity in the brain, leading to various symptoms such as convulsions, loss of consciousness, and altered behaviour. Paediatric epilepsy is a broad term that encompasses a range of seizure disorders affecting children, and its causes can vary, including genetic factors, brain abnormalities, or structural issues.

Diagnosis and management of paediatric epilepsy involve a comprehensive evaluation by healthcare professionals, including neurologists and paediatricians. Treatment often includes anti-epileptic medications, lifestyle modifications, and sometimes surgical intervention in severe cases. Parents and guardians need to work closely with healthcare providers and specialists to monitor and manage the condition effectively, as early diagnosis and appropriate treatment can significantly improve the quality of life for children with epilepsy.

The Symptoms of Epilepsy in Children?

Seizures may manifest with various symptoms, including:

  • Episodic loss of awareness, absences or vacant episodes
  • Involuntary jerking movements (convulsions)
  • Unusual feelings, sensations, and behaviours
  • Periods of confused or odd behaviour
  • Episodes of sudden unprovoked spasms in the body
  • Loss of bowel or bladder control and breathing difficulties are sometimes associated with seizures
  • Bluish tinge on the lips, rapid eye blinking during a seizure
  • Post-event sleepiness is a common occurrence

While these symptoms can vary based on the seizure type, having familiarity with general warning signs empowers parents to recognise and respond effectively. Typically, a seizure can last between 1 to 3 minutes. If the episode persists for more than 5 minutes, call an ambulance immediately.

Please note: if your child experiences a stand-alone seizure, this does not necessarily result in them suffering from epilepsy. Some children who experience a single episode may never encounter another. However, it is recommended to arrange to see a consultant with a specialism in epilepsy.

Our Service

To determine the factors contributing to your child's epilepsy and the occurrence of seizures, our experts conduct a comprehensive series of tests. The diagnostic process involves various examinations and assessments, including:

  • Neurological Exam: A thorough examination to assess neurological function and identify any abnormalities.
  • Imaging Tests: MRI or CT scans are performed to obtain detailed images of the brain, allowing our specialists to detect any structural abnormalities or lesions.
  • Blood Tests: Comprehensive blood tests are conducted to assess general blood levels and identify any abnormalities, including potential issues with blood sugar levels.
  • Electroencephalogram (EEG): This test involves attaching electrodes to the scalp to record the electrical activity in the brain. It helps in evaluating whether the brain wave patterns are normal or if there are abnormalities associated with epilepsy.
  • Lumbar Puncture (Spinal Tap): This procedure measures the pressure in the brain by collecting cerebrospinal fluid. It is a valuable diagnostic tool to identify certain neurological conditions.

What Causes Paediatric Epilepsy?

Paediatric epilepsy can result from various factors:

  • Genetic Factors: Inherited mutations or family history
  • Structural Brain Abnormalities: Birth defects, tumours, or injuries affecting the brain
  • Metabolic Disorders: Conditions like phenylketonuria or mitochondrial disorders
  • Infections: Meningitis or encephalitis
  • Febrile Seizures: Seizures linked to high fevers
  • Neurodevelopmental Disorders: Conditions like autism or cerebral palsy
  • Perinatal Factors: Complications during childbirth or lack of oxygen
  • Immune System Disorders: Rarely, autoimmune conditions affecting the brain

Some cases may have an unknown cause (idiopathic epilepsy). Diagnosis involves medical history, exams, imaging, and sometimes genetic testing. Treatment varies based on the cause and may include medications or surgery. It is recommended to seek medical attention for proper evaluation and management.

Different Types of Epilepsy Seizures

Epileptic seizures can vary in type, determined by the specific region of the brain affected and how they unfold. It is possible for individuals to experience more than one type of seizure.

Common categories of seizures include:

Simple Focal Seizures

Simple Focal Seizures, also known as focal onset aware seizures, originate in a specific area of the brain and affect only one part of the body. During these seizures, a person remains conscious and aware of their surroundings.

Complex Focal Seizures

Complex Focal Seizures, on the other hand, are focal onset seizures that impact consciousness. Individuals experiencing these seizures may exhibit unusual behaviours or engage in repetitive actions. They might not remember the episode afterwards and may feel confused or tired.

Tonic-Clonic (Grand Mal) Seizure

Tonic-Clonic Seizures, commonly known as Grand Mal seizures, are generalised seizures that involve the entire brain. They consist of two phases: the tonic phase, characterised by muscle stiffness, and the clonic phase, marked by rhythmic jerking movements. Individuals often lose consciousness during these seizures.

Absence (Petit Mal) Seizure

Absence Seizures, also called Petit Mal seizures, are brief episodes of staring or blanking out. These seizures primarily affect children and are characterised by a sudden onset and offset without any convulsive movements. The person may not be aware of the seizure during the episode.

Myoclonic Seizures

Myoclonic Seizures involve brief, involuntary muscle jerks that can affect specific body parts or the entire body. These jerks may be mild or severe, and individuals usually remain conscious during the episode.

Tonic Seizures

Tonic Seizures lead to a sudden and sustained increase in muscle tone, causing stiffness. These seizures can affect various parts of the body and may result in a person falling if the legs are affected.

Atonic Seizures

Atonic Seizures, also known as drop attacks, cause a sudden loss of muscle tone, leading to a person collapsing or falling. These seizures can result in injuries as the affected individual loses control of their muscles.

Febrile Seizures

Febrile Seizures typically occur in children and are triggered by a fever, often associated with an infection. These seizures are usually brief and may involve convulsions. While febrile seizures can be alarming, they often do not cause long-term health issues.

Epilepsy Diagnosis

Our team of experienced epilepsy specialists understands the concerns and challenges associated with paediatric epilepsy diagnosis. We strive to provide continuous support to both parents and children, ensuring that they are well-informed and supported throughout the entire treatment journey.

While anyone can experience a single seizure, an epilepsy diagnosis typically requires the occurrence of at least two seizures. The diagnostic process can be intricate, and our neurologists closely examine symptoms before recommending a series of tests.

These can include:

Electroencephalogram (EEG)

Electrodes are attached to the scalp to record electrical signals, helping to evaluate brain wave patterns.

Magnetic Resonance Imaging (MRI)

A non-invasive imaging technique used to assess brain structure and identify any damage.

Video-Telemetry

This test simultaneously records brain waves and captures video footage, often requiring hospital admission for more extended monitoring.

In addition, patients may be asked to keep a diary, documenting symptom patterns, which aids in the diagnosis of specific types of epilepsy. This collaborative approach enhances the accuracy of the diagnosis and ensures a more comprehensive understanding of the patient's condition.

How is Epilepsy Classified?

Epilepsy is classified based on a comprehensive evaluation of various factors by specialists. The elements they examine include:

  • Seizure Types: Identifying the specific types of seizures your child experiences.
  • Frequency of Seizures: Evaluating how often seizures occur.
  • Severity of Seizures: Assessing the intensity and impact of seizures on your child's daily life.
  • Underlying Causes: Investigating potential triggers or conditions leading to epilepsy.
  • Age of Onset: Considering when epilepsy symptoms first appeared.
  • Medical History: Reviewing your child's overall health and any pre-existing conditions.
  • Diagnostic Test Results: Incorporating findings from neurological exams, imaging tests, blood tests, EEG, and other relevant diagnostic procedures.
  • Response to Previous Treatments: Assessing the effectiveness of previous management strategies.

How We Manage and Treat Epilepsy

Effective management of epilepsy involves a tailored approach to address each child's unique needs. Medications play a crucial role, chosen based on seizure type, age, potential side effects, and ease of use. Common forms of medication include capsules, tablets, or syrup. Our specialists work closely with parents to ensure proper administration and understanding of medications.

Beyond medication, our experts emphasise the importance of understanding the condition, its risks, and necessary medications. Regular visits to healthcare providers, adherence to prescribed medication schedules, and routine testing are key components of successful management. Parents are encouraged to educate their children about their condition, and potential triggers to avoid, and ensure a healthy lifestyle, including sufficient sleep.

In some cases, children may not require lifelong medication. Significant improvements in seizure frequency might lead healthcare professionals to consider discontinuation of medication after careful evaluation.

Helping Your Child Live with Epilepsy

Parental support is crucial in helping children live with epilepsy. Key advice includes:

  • Understanding the Condition: Gain a comprehensive understanding of epilepsy, its risks, and required medications.
  • Medication Management: Administer prescribed medications on time and in the correct doses, closely following healthcare professional recommendations.
  • Lifestyle Awareness: Be aware of potential triggers and help your child avoid them, such as ensuring they maintain a healthy sleep routine.
  • Regular Healthcare Visits: Schedule regular visits with healthcare providers for monitoring and adjustments to the treatment plan.
  • Educating Your Child: If age-appropriate, educate your child about their condition, potential risks, and actions to take during and after a seizure.

Treating Paediatric Epilepsy

At Midland Health, we offer a tailored approach to managing each child’s presentation of epilepsy. Medications are a primary focus, chosen based on seizure characteristics, age, and considerations of potential side effects. These medications can be administered orally, such as through capsules, tablets, or syrup. Alternative administration methods are available, and our experts provide detailed information to parents.

Parental involvement is crucial in helping children manage their epilepsy. Regular healthcare visits, adherence to medication schedules, and awareness of potential triggers contribute to effective management. Parents should be vigilant for any side effects and consult with healthcare professionals promptly. Additionally, professional guidance is essential before administering medications for other conditions, as interactions may occur.

Certain instances necessitate immediate medical attention, including the first seizure, repeated seizures, increased frequency, uncertainty about a seizure occurrence, pregnancy, diabetes, injury, or worsening seizure patterns over time. Parents are encouraged to communicate with healthcare professionals promptly in such situations.
