How We Can Help

Our clinician will listen to your concerns and examine the lumps in your neck and throat. We may recommend further testing and examinations, such as a nasoendoscopy or ultrasound imaging.

This clinic focuses on diagnosing and managing lumps that form in the thyroid gland (thyroid nodules) and other causes of a lump in the neck.

Because thyroid lumps can be benign (non-cancerous), and less commonly more serious, a careful evaluation matters.

Who This Service May Be Suitable For

If you’ve noticed a thyroid lump, or a lump in your neck that you’re not sure is related to the thyroid, it’s sensible to have it checked.

This clinic may be suitable for adults who:

  • Have a new or persistent neck lump or neck swelling
  • Have a known thyroid nodule and want specialist review
  • Have symptoms that may relate to a thyroid lump in the neck, for example, a visible swelling or a change you can feel when swallowing

Possible Causes Of Neck And Thyroid Lumps

A lump on the neck can have many causes, and a thyroid lump is only one possibility. The main cause is unknown, but thyroid nodules and thyroid enlargement are more common in women than men and increase in frequency with age. The thyroid gland can also enlarge during pregnancy, due to hormonal changes.

If you have a nodule or swelling in your neck you should see your doctor as soon as possible. Your doctor will assess the swelling and refer you to a specialist clinic if necessary. Most nodules and swellings are harmless, but it is always best to have them checked promptly in case they are among the few that are cancerous.

Solitary nodules are most commonly benign and very often can be left untreated. If a cancer cannot be excluded by investigations, surgery is usually recommended. If the nodule is ‘hot’ (i.e. it produces too much thyroid hormone) then it can be treated either by drug therapy, radioactive iodine treatment or sometimes surgery.

A multinodular goitre is common and usually does not need an operation unless you have problems with swallowing and/or breathing, or if you feel the goitre is unsightly. If the thyroid gland is growing rapidly or one or more of nodules raises concern following investigation, then we may also recommend surgery.

This is often caused by autoimmune thyroid conditions such as Hashimoto’s thyroiditis and Graves’ disease and can be associated with an over- or an under-active thyroid.

Sometimes a multinodular thyroid grows down behind the breastbone. It can constrict or squash the windpipe (trachea) and the large veins in the neck and/or the gullet (oesophagus) because it is in a fixed bony space. In this situation, surgery may be considered. Most retrosternal goitres grow slowly over many years.

Types of Thyroid Nodules

These are benign thyroid neck lumps which can be solitary or found in a multinodular goitre. They can usually be observed without the need for surgery.

This is also a benign lump, but is harder to distinguish from a cancer by scans and biopsy. It is, therefore, usually removed surgically so that its benign nature can be confirmed by close examination under the microscope.

This is a swelling that contains fluid, which is treated by removing the fluid through a fine needle. If this fails, then surgery may be advised.

Thyroid cancer is rare and has a high cure rate. The treatment for thyroid cancer is surgery to remove most or all of the thyroid gland, followed by radioactive iodine ablation in some cases. You may need to take levothyroxine (synthetic thyroid hormone) in a slightly higher than normal dose to replace your body’s thyroid hormone production and to stop the cancer from recurring.

Neck Lump or Swelling

If you notice a lump or swelling in the neck, it could be a sign of an enlarged thyroid (goitre) or thyroid nodules, which require professional evaluation to rule out conditions such as thyroid cancer or benign growths.

Many thyroid nodules and swellings turn out to be harmless. Our goal during assessments is to identify the smaller number that need closer follow-up or treatment.

Your consultant will recommend testing and treatment based on the most appropriate course of action.

What To Expect From Your First Appointment

You’ll be seen by a senior consultant, with time to talk through what you’ve noticed and what’s worrying you most.

Your appointment may include a physical examination of the neck and recommendations for further testing where appropriate. We may carry out a throat examination using a nasoendoscope, and you may need an ultrasound scan to assess the structure, size and features of the nodules. 

Our clinicians provide recommendations based on your history and examination. We’ll explain what each test is for, and only recommend what’s appropriate for your situation. Tests may include:

  • Blood tests to check whether the thyroid is over- or under-active
  • Fine needle aspiration cytology (FNAC), in which we take a small sample of cells for lab analysis
  • CT scan in selected cases, for example, to map a goitre that extends behind the breastbone
  • Nuclear medicine scan to assess “hot” or “cold” nodules

Results And Next Steps

Waiting for answers is often the hardest part, so we’ll be clear about what you can expect and what happens next.

Some findings can be discussed on the day, for example, during or after an ultrasound. Lab-based results usually take longer and are reviewed at a follow-up appointment. 

If your case needs wider input, your consultant may review findings with radiology and pathology colleagues as part of a multidisciplinary discussion.

Possible Procedures And Treatments

If treatment is needed, we’ll talk you through options calmly, including what can be monitored and what may need active treatment.

Depending on the cause, management may include:

  • Monitoring with repeat assessment/imaging
  • Medication where appropriate; for example, thyroid hormone replacement or antithyroid medication
  • Referral for specialist procedures such as thyroid surgery or radioactive iodine when indicated

  • Is a thyroid lump always serious?

    Many thyroid nodules and swellings are benign, but it’s still worth getting checked so you know what you’re dealing with.

  • What’s the difference between a goitre and a thyroid nodule lump?

    A goitre is an enlarged thyroid (which may feel smooth or lumpy), while a nodule is a distinct lump within the thyroid.

  • Will I need an ultrasound?

    Not always, but ultrasound is commonly used to assess thyroid lumps and neck swellings in more detail.

  • What is fine needle aspiration cytology (FNAC) and does it hurt?

    FNAC uses a very thin needle to take a small sample of cells from a thyroid nodule for lab analysis; discomfort is usually brief.
